- Faith Formation for Children
  
  Welcome to Holy Family's Faith Formation Program. We seek to provide Catholic youth of all ages with orthodox instruction that compliments and supplements the formation they receive from their parents, the first teachers of their children's Catholic faith. Please fill out and submit the form below with payment either online or sent to the church office. Holy Family registered parishioners will have priority for enrollment.
  
  Important notes to keep in mind:
  Second grade is preparation for reception of First Holy Communion. A copy of Baptism certificate will be required.
  If your child wishes to receive First Communion and is older than 2nd grade, please select the grade level corresponding with your child's age.
  Ninth grade starts the first of two year's preparation for Confirmation. A copy of Baptism certificate will be required.
  If your child wishes to receive Confirmation and is older than 9th/10th grade, please select either 9th (1st year) or 10th grade (2nd year) according to your child's progress in Confirmation preparation.
  ALL CCD grades meet on Sundays, 10:45 - 11:45 AM in the Ministry Center beginning September 8th.
  Catechesis of the Good Shepherd, Level 1 (ages 3-6) meets on Wednesdays, 9:00-10:30 beginning September 11th

  All CCD grades and CGS will have one class session per year that reviews the Diocesan "Circle of Grace" material. For more information, please visit the Diocesan Safe Environment webpage. Parents will be notified ahead of time and have the choice to opt out.
